For overnight camps, specifically sports camps, there are required forms to be completed. These include, but may not be limited to, Immunization Records and a Health Form. A Parental Release Form may also be asked to be completed by some programs and all sports camps. These forms are submitted in the best interest of the participants and the University.
New Jersey Youth Camp Standards (N.J.A.C. 8:25) require campers to be immunized with the vaccinations required for child-care center, preschool or school attendance as appropriate for the camper's age, according to the immunization schedule found in N.J.A.C. 8:57-4. An immunization schedule can be found at www.state.nj.us/health/cd/imm_requirements/.
Princeton Sports Camps require that participants bring signed copies of both the Parental Release Form and Health Form, as well as a copy of their most recent immunization records to camp check-in.
